New Patient Forms
New patient forms help us get to know you and your needs. By downloading and filling them out before your appointment, we can streamline your visit and get a better idea of how we can best serve you and your child.
For your convenience, all new patient forms will be sent securely via email or text before your child's appointment. This includes the New Patient Registration, Medical History, Dental History, HIPAA Acknowledgment, Financial Policy, Consent for Treatment, and any additional forms specific to your child's visit.

As a courtesy, our team will verify your insurance benefits, provide an estimate of your expected costs, and file your claims for you. While we do our very best to provide accurate information, please remember that your insurance company makes the final decision regarding coverage and payment.
We also encourage our families to become familiar with their own dental insurance benefits, including deductibles, annual maximums, waiting periods, exclusions, and covered services. Understanding your plan helps ensure we're all on the same page and can prevent unexpected surprises.
